HPDP Assistant (Health Promotion Disease Prevention) Full or Part Time
Assists HPDP staff with community-based programs and other HPDP clinic projects to include data management, patient management and event preparation. Performs basic health assessments, knowledge and behavioral assessments and assists in leading experiential learning activities. Processes patient referrals through Electronic Health Record (E.H.R) and Patent Registration System (RPMS) and schedules appointments as appropriate. Completes patient appointment/event reminder calls/letters as appropriate. Maintains HPDP databases. Assists patients in completing new and updated forms for the RPMS Patient Registration System. Maintains current documentation for HPDP programs
High School diploma or GED, computer experience or training & phone experience required. This position is funded for one year with the possibility of future years based on grant renewal.
